BACKGROUND: Coronavirus disease 2019 (COVID-19) is a related to coronavirus (SARS-CoV-2) which categorized as Nidovirales order, family beta-coronaviridae. infects mainly the lung well other organs out of lung. Extrapulmonary infection includes neurological such central nervous system, peripheral and musculoskeletal with various symptoms. Recent study reported that 30% patients had COVID-19-associated skeletal muscle injury.
 CASE PRESENTATION: A 78-year-old male history stroke Type 2 diabetes mellitus came emergency department fever, shortness breath, decrease consciousness, joints, muscles pain. Chest X-ray showed consolidation in mid-lower bilaterally, prominent bronchovascular markings, polymerase chain reaction COVID-19 result comes positive times 10 days interval. The patient treated antibiotics, Avigan, azithromycin, high-dose intravenous Vitamin C. In 3rd ward day, still has short breathiness. However, fever consciousness improve, but complaining pain shoulder joint, arm muscles, knee joint. On 11th inpatient obtains full consciousness; dyspnea improved, no however, complains joint At end period seem be improved; not improved an analgesic. back his home good condition.
 CONCLUSION: may develop symptoms injury or myalgia, this our first case complicated injury.
